Spanish) is intended to help family caregivers prepare for a
loved ones hospital discharge. Discharge planning is a process,
not a single event, which begins on admission to the hospital. This
guide will help you understand what to expect. You may want to print
this guide and share it with other family members or keep it as
a reference.
