A Family Caregiver's Guide to Hospital Discharge Planning

   

This guide ( in English, in Spanish) is intended to help family caregivers prepare for a loved ones hospital discharge. Discharge planning is a process, not a single event, which begins on admission to the hospital. This guide will help you understand what to expect. You may want to print this guide and share it with other family members or keep it as a reference.

The authors of A Family’s Guide to Hospital Discharge Planning are Gail Gibson Hunt, Executive Director for the National Alliance for Caregiving and Carol Levine, Director of United Hospital Fund’s Family and Health Care Project.
