Daily task appraisal

   

If you aren't sure what your caregiving needs are, or you think the person you are caring for may need additional assistance, we suggest you start with this short appraisal of the functioning and independence of your older adult.

 

Caregiver self-assessment

   

This tool is indended to help you, the caregiver, identify your level of stress, and seek appropriate help both for you and your dependent.

Use this checklist to improve home safety and help prevent accidents.

 

Home safety information

   

While a safe environment is good for everyone, it is especially important as people get older. As the body ages, changes occur that make seniors more prone to accidents in their homes. With some adaptations, often minor, the home can become safer. The goal of this booklet and checklist is to improve the ability of family caregivers and professionals in the aging network to support aging in place by reducing safety risks in the home for seniors and those with cognitive impairments.

 

Caregiving record

   

Once completed for the person you are caring for, this personal caregiving record will help you remember and help caregivers know the older adult and ensure consistent care by multiple caregivers.

 

Medication list

   

Use this to keep track of medication used by the care-receiver.

A list of important paperwork/documents that caregivers should have, providing information about the person you are caring for.

 

Support groups

 

 

A list and information about support groups and services available for caregivers in New Jersey. It is important that caregivers develop a support network and this can help you find local support.
