  • Assisted Living, Nursing Home, and other Long Term Care Settings

    On this site you can find an assisted living facility, nursing home, or other long term care setting in a specific community. Search by county, name of facility, facility type and/or funding source. Other long term care settings include, but are not limited to: adult family care; comprehensive personal care homes; assisted living programs in subsidized senior housing; specialized programs such as behavioral management or ventilator care facilities; and others.
