PURPOSE

The purpose of the ELPR Program is to provide financial assistance to relocate a child and his/her family when
that child has tested positive for lead poisoning and that child is removed from his/her dwelling unit in connection
with an order to abate a lead-based paint hazard.

WHO CAN APPLY?
Application intake is currently limited to the head of household with a qualifying child who is hospitalized due to
lead poisoning and/or has an elevated blood lead level of 20ug/dL or greater.

WHAT DOCUMENTATION IS REQUIRED?

  • A completed application
  • A copy of the Abatement Order/Notice to Abate on the primary residence of the qualifying child
  • A statement issued by the Health Official, primary care physician or attending physician that the health of
    the child dictates that he/she must be relocated from their primary residence to prevent ongoing exposure to
    lead-based paint hazards.

HOW LONG DOES IT TAKE TO QUALIFY AN APPLICANT?
Applicants can be qualified the same day if all documentation is submitted. A long-term relocation plan must be developed following eligibility determination.

WHEN SHOULD THE HEAD OF HOUSEHOLD APPLY FOR ASSISTANCE?
As soon as possible following notification that their household must be relocated.

WHERE CAN THE FAMILY BE RELOCATED TO?
Each case differs according to the needs of the family. We can provide assistance for placement in shelters,
hotels, and apartments on short or long-term leases.

WHAT IS LONG-TERM RELOCATION PLANNING?
Long-term relocation planning means that the ELPR Program will work with case managers and the family to develop
a housing plan that includes immediate relocation needs combined with a long-term housing strategy. For example,
the ELPR Program pays 4 weeks stay in a hotel awaiting return to the primary residence following lead abatement.
The ELPR Program pays a hotel stay during the applicant’s housing search and pays a rental subsidy when housing
has been identified.
