application deficiencies
To help ensure complete permit application submittals, which help
speed processing time, the following is a list of common application
deficiencies for the various permit types issued by the division.
Treatment Works Approval (TWA) Application Deficiencies
Administrative Deficiencies
- The applicant listed on the TWA-1 Form is incorrect. The applicant
should be the entity that will eventually own and operate the
proposed treatment works.
- Signatures on the TWA-1, WQM-003, and WQM-006 forms are older
than 1 year and/or are not original.
- The application fee is not included with the application or
is calculated incorrectly.
- Notifications to the Local Planning Board and Environmental
Commission are not included (certified mail - return receipt requested).
- Engineering documents are not signed and sealed.
Technical Deficiencies
- Gravity sewer lines are designed below minimum slope requirements
or are oversized to circumvent the need for a pump station and
force main.
- The Engineer's Report (Form WQM-006) and the final construction
plans are inconsistent.
- The site plan is incomplete (for example, plans do not show
existing sanitary sewer lines, water mains, storm sewers, stream
crossings, etc.).
- The design flow calculation is inconsistent with Department
requirements and/or greater than the amount of flow approved by
the owner of the receiving wastewater treatment facility.
- The engineering design deviates from the Department's requirements
and adequate justification is not provided.
Treatment Works Approvals are issued by the Bureau of Financing and Construction Permits at (609) 984-6840, and by the Bureau of
Permit Management (609) 984-4428.
NJPDES Significant Indirect User (SIU) Permit
- A statement of Consent (WQM-003) is not obtained from the
affected local authorities.
- Signatures on the NJPDES-001/Form L are not dated and original.
- An accurate process flow diagram with water balance (line
drawing) as required under item 3. of NJPDES-001/Form L is missing.
- Production information (if required under item 7 on NJPDES-001/Form
L) is not accurate or does not reflect any planned future activities
at the facility.
- Data for all required pollutants is not listed under items
10A, 10B, 10C, and 10D on NJPDES-001/Form L.
- Required attachments under items 11, 12, and 13 of NJPDES-001/Form
L are not included.
- Applicable Federal Categorical Pretreatment standards are
not identified.
- A Sewer Use Ordinance, identifying local effluent limits or
agreements with the receiving local authority, is not included.

NJPDES Discharge to Surface Water Permit-Municipal
- Items requested on the application forms are left blank. All
information requested should be provided. If a particular item
does not apply, please indicate "not applicable."
- All signatures on forms are not dated.
- The design flow should be listed under item 8a, NJPDES Form A, not the metered effluent flows which are entered in 8b & 8c.
NJPDES Discharge to Surface Water Permit-Industrial
- Data for all pollutants are not listed on NJDEP Form C.
- Plot plans don't clearly show all of the facility's outfalls.
Questions regarding the submission of Surface Water Discharge
Permits should be directed to the Bureaus of Point Source Permitting
1 at (609) 633-3869 or 2 at (609) 292-4860.
NJPDES - Discharge to Ground Water (DGW) (New Construction)
- The applicant listed on the NJPDES-1 Form is incorrect. The
applicant should be the entity that will eventually own and operate
the wastewater treatment system and disposal area.
- Signatures on the NJPDES-1 and WQM-003 are older than one
year and/or are not original.
- Proof that the proposed project is consistent with the applicable
Water Quality Management Plan is missing. Consistency Determinations
are issued by the Office of Land and Water Planning (609-633-1179).
- The soil borings are not advanced to the required minimum
depth of 20 feet or to bedrock, whichever is shallower.
- Permeability determinations for each disposal area are inadequate.
(Some applicants fail to document the methodology, field notes
and results of permeability tests conducted on-site.)
- The ground water mounding analysis is deficient or missing.
The model must be run assuming peak flow during seasonal high
water table conditions. The applicant must submit the method and
supporting documents for a model which demonstrates adequate hydraulic
performance of the proposed disposal areas.
- The applicant is not planning to achieve ground water quality
standards prior to discharge, and has not provided an adequate
dilution model. The model must quantify the characteristics and
delineate the boundaries of the wastewater plume, while accounting
for initial piezometric conditions and background water quality.
All pertinent hydrogeologic characteristics of the site should
also be submitted.
NJPDES-DGW Permits for facilities which have an operational
discharge of sanitary or industrial wastewater are issued by the
Bureau of Nonpoint Source Pollution Control which can be reached
at (609) 292-0407.
