Clean Watersheds Needs Survey (CWNS) Forms

CWNS 2012 Survey Forms:
Each forms listed below are in two formats - MS Word and Adobe PDF. Use either word or pdf form and submit the completed form to list needs for your facility or project in the 2012 Clean Watersheds Needs Survey.

Small Community Survey Form (for Population < 10,000) - Use this form if the total population of community that the facility or project will serve is less than 10,000.
