Initial Application Fees

An initial application fee of $40.00 is required for each new x-ray machine registration.

Registration Fees

The first year registration fee is a prorated amount of the annual fee based on the number of months remaining before the next billing cycle.

Facilities are invoiced annually for all active x-ray units. Invoicing occurs over a 4 month period and all registrants have 60 days to pay their annual registration renewal fees.

• Facilities whose business name begins with A-F are invoiced on or about July 1
• Facilities whose business name begins with G-L are invoiced on or about August 1
• Facilities whose business name begins with M-R are invoiced on or about September 1
• Facilities whose business name begins with S-Z are invoiced on or about October 1

Please note that failure to pay all registration fees in a timely manner may result in assessment of late fees and penalties.
