[SRRA]: Updated Forms and Case Inventory Document Posted to Site Remediation Program Web Page

The New Jersey Department of Environmental Protection (Department) announces the availability of a revised Case Inventory Document (CID), revised forms, and accompanying instructions. The forms were revised with stakeholder input from representatives of the License Site Remediation Professional Association and Environmental Business Council. There are no new requirements. The changes are intended to make the forms more user-friendly through improved instructions, consolidating and removing some questions, and the use of consistent terminology among the forms.

The Department has also updated the June 2013 Policy Statement "Interpretation of SRRA Requirement to Complete the Remedial Investigation by May 2014" to reflect application of this policy to all cases, irrespective of the timeframe for completing the remedial investigation. The new policy is entitled "Policy Statement – Interpretation of Completing a Remedial Investigation at a Contaminated Site," and may be found on Department's web page at

The following is a brief overview of the changes to the CID and forms:

CID:
The intent of the new spreadsheet is to obtain consistent information by replacing written narrative entries with dropdown selections. This has increased the number of overall columns but completion is simplified by the use of dropdown fields. The design of the new CID is aimed to reduce the frequency of phone calls that have occurred between the Department’s staff and LSRPs. Also, the changes have been made to accommodate the ability to upload the CID when additional DEP Online Services become available. The Department encourages LSRPs to begin using the new version of the CID following its posting. Be advised that the new CID must be used when utilizing any DEP Online service that requires the uploading of a CID. The first DEP Online service requiring a CID is the Remedial Investigation Report service which should be available later this summer.

Preliminary Assessment/Site Investigation Form:
The information requested in this form has been reorganized. There is now a General Section with questions that would apply to a PA or an SI submission, a PA Section and an SI section. A new checkbox was added for the PA/SIs submitted pursuant to N.J.A.C. 7:26E-3.9 of the Technical Requirements for Site Remediation, specifically when the PA/SI is conducted to determine if there are off-site sources of soil/ground water contamination.

Remedial Investigation/Remedial Action Workplan/Remedial Action Report Forms:
Revisions to these forms include the consolidation and removal of questions that are no longer necessary for these remedial phase reports.

Response Action Outcome Form:
Revisions to this form include the consolidation and removal of questions that do not relate directly to the RAO. Redundant questions between the remedial phase forms that are submitted concurrent with the RAO form were eliminated.

Other Revised Forms:

Remedial Action Permit Application Instructions for Ground Water and Soils
Minor modifications were made to the instruction to improve clarity such as adding a link to the Quick Reference Guide for permits, Monitored Natural Attenuation Technical Guidance and added GIS instructions.

Remedial Action Protectiveness Certification Form for Ground Water and Soils
Revisions to these forms include the consolidation and removal of questions that are no longer necessary. In addition, there are three new sections added: Section I. Vapor Intrusion, Section J. Financial Assurance, and Section L. Subsurface Evaluator Information and Statement.

Also, in an effort to streamline the implementation of SRRA the following forms have been removed and are no longer required:

  • CEA/Well Restriction Area (WRA) Spreadsheet
  • Potable Well/Indoor Air Sampling Notification form and Spreadsheet
