Attention Registered Back Office/Service Facilities in New Jersey

Your registration is set to expire on August 31, 2016 and needs to be renewed for an additional 2-year period until August 31, 2018.

(Not sure if this applies to you? View regulations: N.J.A.C. 3:1-18.1 et. seq.)
Here’s what you need to do to renew your registration:
1. Complete the application for renewal for each back office/service facility you need to register.
2. Provide a check for $500 for each facility to be registered.  The check should be made payable to:  “Treasurer, State of New Jersey”
3. Mail the application(s) and check to

Bank Applications 5th Floor
PO Box 040 (for regular mail)
              or
20 West State St. (for overnight delivery)
