Municipal Questionnaire for the Flood Mitigation Plan
This questionnaire asks for information on your community and its flood hazard; please fill it out as completely as possible. We encourage you to discuss this questionnaire with others in your community to provide the most accurate information. This can include your Local Emergency Management Coordinator, Public Works Supervisor, Floodplain Administrator (as identified in your Flood Damage Prevention Ordinance), Building Code Official, Planner, Engineer, Administrator/Mayor, Residents, etc.
This questionnaire should be the initial focus of your newly formed local flood mitigation planning committee, and its completion is necessary to communicate local knowledge and conditions to the overall regional plan. In addition to submitting the questionnaire, please also provide a copy of your municipal Flood Damage Prevention Ordinance, a tax map delineating the areas impacted by each flood event, and a map of your evacuation routes. If you are submitting the questionnaire electronically, please feel free to submit the ordinance and maps either by email or under separate copy by mail. We will need to get this completed questionnaire back from your community by January 26, 2007 to remain on schedule.
