1. Download the School Registration form (Microsoft Word document). Complete both sides of this form in ink.
2. Please make all checks payable to: Pennsylvania Parks and Forestry Foundation
3. Send registration form to Youth Summit, Delaware Canal State Park, 11 Lodi Hill Road, Upper Black Eddy, PA 18972-9540
4. Please list school or student name on check, and please notify us of sponsors or other 3rd party payments
5. Registration forms are due by April 20. (fax 610-982-0160)
6. Waivers and payment must be presented prior to program on April 29.
