Attach a copy of the notification from your district of residence stating that your child may participate in the school choice program. The regulations provide that if you do not receive a notification from your district of residence, your child may participate in the school choice program.
If you did not receive a notification from your district of residence, make a note on the application that no notification was received.
NOTE: If your child has been placed on the waiting list at the choice district, you may still receive a Notice of Acceptance during the second application cycle.
