Cooperative Agreement with CDC for State Based Programs to reduce the burden of Diabetes

Purpose: 
The Council aids in the coordination of diabetes related efforts of the public health system with private health care providers and payers with all appropriate governmental, voluntary, professional and academic institutions in order to reduce the burden of diabetes in New Jersey.
