The ACF data originated from a database, which contains attribute information for all licensed ambulatory care facilities in New Jersey. This database is maintained by the Certificate of Need Acute Care Licensure Program (CN), NJDHSS. In June 2002, CN prepared a spreadsheet of New Jersey's ACFs for the Center for Health Statistics (Center).
The Center geocoded the ACF addresses in ArcView 8.1 using the Census TIGER 2000 street file (New Jersey), which was obtained from the New Jersey Department of Environmental Protection. After batch and interactive geocoding, 527 addresses were matched, and 1 was unmatched. The unmatched address belongs to the Central Jersey Ambulatory Surgical Center, 511 Courtyard Dr, Hillsborough, NJ, 08844. Despite numerous attempts to identify the correct address of this ACF, a location could not be found. Therefore, please note, that this shapefile contains attribute data for 528 ACFs, but the spatial location for only 527 ACFs.
The New Jersey Department of Health & Senior Services (NJDHSS) provides these geographic data "as is." NJDHSS does not guarantee the accuracy of information contained in the geographic or attribute data. Additionally, NJDHSS makes no warrenty regarding the data's fitness for any particular purpose. The burden for determining fitness for use lies entirely with the user.
After batch geocoding, the "match interactively" process was used to obtain a match for those addresses that were originally labeled as "unmatched." For those records with no potential match candidates or candidates with low match scores, attempts were made to determine the accuracy of the address. These attempts included one or more of the following: 1) entered facility name and address into an Internet browser to located facility website or other legitimate listing, 2) entered facility address into the web-based mapping application, <http://www.mapquest.com>, or 3) called the facility directly and asked for location information. After geocoding, all but one facility address were sucessfully matched. As a result, this shapefile contains attribute data for 528 ACFs, but the spatial locations for only 527 ACFs. Data file is complete as of 6/11/02.
