Abstract:

This shapefile contains all primary Federally Funded / Qualified Health Migrant Centers (FQHC) in the state of New Jersey. Satellite centers located within public schools are not included. FQHCs provide much needed care to the most impoverished citizens in the state. The comprehensive services of each center are tailored to the physical, psychosocial, nutritional, and health educational needs of their communities. The information included with this file originated from the New Jersey Primary Care Association, Inc., Princeton, NJ.

Purpose:

This shapefile was created in response to the need for a geographic file of New Jersey's federally qualified health centers.

Supplemental_Information:

FQHCs are represented by the New Jersey Primary Care Association, Inc., and the New Jersey State Office of Rural Health. FQHCs are federally funded/qualified by Sections 330/329 of the United States Public Health Service. All FQHCs are staffed with well-qualified Board-certified or Board-eligible physicians representing a broad array of medical specialties and are subject to stringent Federal regulations in all operational areas. Most FQHCs are located in high-density, urban areas and in rural areas with a large, diverse base of ethnic and minority residents.

Process_Description:

In April 2003, the Center for Health Statistics, NJ Department of Health Senior Services (NJDHSS), obtained a hardcopy listing of New Jersey federally qualified health centers. The Center entered facility address and contact information into a MS Access database table, and imported table into ArcView 8.2. The Center then geocoded the facility addresses listed using the geocoding service, Census TIGER 2000 street files (New Jersey).
