Occupational Disease Reporting Requirements and Forms

Reports of work-related diseases, injuries, and poisonings trigger public health follow-up by the Occupational Health Surveillance Program. This includes an interview with the reported individual and worksite contact to ensure that co-workers are not also at risk.
 
Reporting is vital to preventing further disease and is required by the New Jersey regulations listed on this page. Reportable work-related conditions include: silicosis, work-related asthma, asbestosis, poisonings due to heavy metals and pesticides, work-related injuries in children under the age of 18, work-related fatalities, and occupational dermatitis. A complete list of reportable conditions can be found in the regulations and on the reporting forms.
