Planning for Long-Term Care in the Community

Long term care planning often occurs as a result of a crisis, after oneself or a loved one becomes hospitalized and needs extended care.

Many people believe that long term care can only be provided in nursing homes. Today, however, there are other places where individuals, who cannot live independently, can receive the assistance they need. Community Choice counselors are available to help nursing home residents and hospital patients investigate their options.
