Adult Day Health Services was developed as a community-based option
in April, 1977. Adult Day Health Services is defined as follows:
A program of medically supervised, health-related services provided
in an ambulatory care setting to persons who are non-residents of
the facility, who do not require 24-hour inpatient institutional care
and yet, due to their physical and/or mental impairment, need health
maintenance and restorative services.
Eligibility
Individuals who request Adult Day Health Services must meet financial
and medical requirements for Medicaid coverage. The eligibility criterion
for Medicaid coverage is stated in N.J.A.C. 8:86-1.4 (b). Also, Adult
Day Health Services centers serve individuals who privately pay.
Center Locations
Adult Day Health Services centers are located in nursing homes, affiliated
with hospitals or are freestanding community- based programs. The
New Jersey Department of Health and Senior Services must license centers
prior to becoming a Medicaid provider.
Services
Centers are required to provide medical, nursing, social, personal
care and rehabilitative services, as well as a nutritious midday meal,
activities and transportation to and from the center. Services are
provided five days a week, approximately five to seven hours during
the day.
Center Staff
Adult Day Health Services center staff include a Medical Director,
Program Director, Registered Nurse, Social Worker and Recreational
Therapist. Staff-participant ratio is required at a minimum to be
one staff to every nine participants.
To Apply for Services
An individual interested in participation in an Adult Day Health Services
center can apply for admission at the center. Adult Day Health Services
center staff will assist in the application process.
Administration
The Department of Health and Senior Services monitors each center. Further information is available by calling the Office of Community Programs at (609) 633-8406.
