New Survey Instructions
  1. All surveys have been revised substantially as of June 2008, including Spanish versions. Please destroy all other surveys. Print and distribute the June 2008 version only. Current versions are on this website.
  2. The survey administration period is June 30, 2008 until March 1, 2009. If you have summer program orientations coming up, you can begin surveying then.
  3. Adequate response rate for surveys administered remains at 80% for students and 50% for parents, based on your Year 4 (08-09) GEAR UP enrollment, excluding additional students.
  4. Additional students should only be given the Additional Student Survey. You will no longer administer the regular Student Survey to them.
  5. Surveys should be submitted with Final Performance Report in November (at least 75%) and the Annual Performance Report in March (the remainder).

Mailing Addresses: There are 2 mailing addresses. Overnight mail will not be delivered to the PO Box and vice versa.

Thank you all for your assistance for the External GEAR UP Evaluation.

Scholarship Deadlines: Eligible students must complete and submit scholarship forms to CHE on time! If students do not adhere to the deadlines, they will forfeit payment for the current semester. Students who have not filed the FAFSA cannot receive the scholarship.

  • June 1 for next academic year
  • November 1 for spring semester only

Do not send forms until the students make their final decisions. Do not leave the college name blank or write multiple schools. We use the information to determine where to send the scholarship checks. You now should have a report in your database to show students who are eligible for the scholarship.

New this year: Graduating high school seniors must also submit a high school transcript request form. This information will confirm course completion of 12th grade students for the annual performance report.

Eligible students entering college submit to NJ GEAR UP once per year or if they transfer to another eligible institution mid-year. Students who plan to enroll at the same school for the entire year do not have to submit a new form in November.

This form tells CHE where to send scholarship funds. Students must include the college where they plan to enroll.

High school juniors and seniors complete this form to allow recruiting contact by the Educational Opportunity Fund (EOF) program.

DO NOT include with the scholarship forms or submit for 12th grade students after November 30.

Mentor Application Ongoing Provide information for mentor screening and hiring process. The form can be completed online.
