The Supports Program is a Medicaid waiver program through which adults with intellectual and developmental disabilities who live in a non-licensed setting can access Division-funded services. 

The Supports Program is part of the Divison's system-wide reform efforts begun in 2012, and is not yet fully implemented and available to all individuals.

To be enrolled on the Supports Program, an individual must:

(1) be 21 years of age or older
(2) meet the functional criteria for service eligibility through the NJ Division of Developmental Disabilities (DDD)
(3) be Medicaid eligible and maintain Medicaid eligibility

Supports Program Policy Manual
The Division is in the process of developing comprehensive policy manuals for the Supports Program and the Community Care Waiver that will be based on the format of the Interim Policy Guide to Support Coordination. We anticipate the draft policy manuals will be released by the end of February 2015.

Services Available Through the Supports Program

Services available through the Supports Program include Employment/Day Services and Individual/Family Supports, and Support Coordination. For a complete list of services available through the Supports Program, please see the Supports Program Draft Service Definitions and Qualifications.
Support Coordination

Support Coordination is a Division-funded service provided in the community by independent, Division-approved Support Coordination Agencies.
Every individual enrolled on the Supports Program selects (or is assigned by the Division to) a Support Coordination Agency. The Support Coordination Agency then assigns a Support Coordinator to initiate the Person-Centered Planning Process with the individual and his/her family.
