Applicant must hold a valid, unrevoked license as a Certified Public Accountant and submit the required application for Public School licensure and the required application fee (see full requirement for C.P.A. license) which must be held. Applicant of good moral character, must hold valid, unrevoked CPA license from another State/jurisdiction with substantially equivalent education/experience/examination and re-exam requirements as NJ's or applicant has held valid license issued by another jurisdiction for 10yrs or, if license held for only 6yrs, 4yrs of his/her practice must have been in government, industry or education or if licensed by foreign authority, he/she has met all educational, experience & exam requirements of that authority, and successfully passes qualifying exam in national standards. Candidate who has never been licensed in any other jurisdiction but have taken the CPA exam in another jurisdiction and achieved passing grades and with by which to be licensed in NJ. -candidate must meet educational requirements including a bachelor's or higher degree from accredited institution ( at least 150 hrs of education). Applicant must fill out reinstatement applicaton. -pay all past delinquent license renewal fees, -pay current triennial license renewal fee, -pay reinstatement fee, -submit copy of most recent peer review acceptance letter, -if reinstating as limited liability partnership or corporation, must be registered with the NJ Division of Revenue. Applicant must fill out reinstatement application and provide list of every job held during period of expired/inactive-unpaid licensure -pay all past renewal fees -a reinstatement fee and any additional penalties imposed by Board -provide proof of completion of 120 continuing professional educational credit hrs during the 3yrs prior to reinstatement as per regulations. Applicant must be 21yrs old -provide affidavits from 2 people attesting to applicant's good moral character -Bachelor's degree from accredited college/univ. -Certified transcript from accredited school of acupuncture confirming diploma from that school (see NJAC 13:35-9.3 for alternate pathway to certification). Applicant must be at least 18yrs old-good moral character -hold H.S. diploma or equivalent -no conviction of crime of 1st,2nd or 3rd degree in 10yrs prior to submission of app.-no convict of crime of 4th degree of having provided alarm services while unlicensed -with period immediately preceding submission of app., having obtained 4yrs of practical, hands-on experience in installation, alteration or repair of wiring for the alarms, burglar alarms and/or electronic security systems (see NJAC 13:31A-3.1(a)7i-iii for details re: other means of meeting experience requirement) -successfully complete burglar or fire alarm exam. Applicant must submit required upgrade application -submit transcript from an accredited college/univ. to show that he/she has earned a master's degree in an addictions or counseling related area that included a min. of 18 graduate semester hours in specified areas of study (see NJAC 13:34C-2.2(b)1) -submit to criminal history record background check. Applicant must be at least 18yrs old -good moral character -has not been charged by NJ State Board of Architects or any other licensing authority with a violation of any statute or regulation related to practice of architecture proof of having met the education, training and exam requirement in that other jurisdiction that are substantially equal to those required in NJ provides satisfactory proof of competency (see above-cited reg.). Within 3yrs of the expiration of a license, an applicant for reinstatement may have his/her license renewed by paying a reinstatement fee and a restoration fee. If the license expired more than 3yrs ago, the license will be renewed only by complying with the provisions of the Athletic Training Licensure Act regarding initial licensure. Applicant must be at least 18yrs old -Good moral character, -H.S. diploma successful completion of athletic training program at college/univ. approved by board which led to awarding a bachelor's degree. -Baccalaureate curriculum shall include courses of study in biophysical sciences for the use of physical agents and medical-surgical techniques as related to athletics. Applicant must hold a Bachelor's or master's degree in audiology or speech pathology or both from an accredited college/univ -Serve a clinical internship under direct supervision of a licensed audiologist or speech pathologist -Applicant's doctorate in either area need not serve an internship -Complete 25hrs of supervised observation of evaluation and treatment of children or adults w/disorders of speech language or hearing -Passing grade on National teacher's exam in audiology and/or speech pathology -Submit to criminal history background check. Applicant licensed in another state or jurisdiction with substantially similar licensing requirements shall provide proof of attaining a doctoral degree in audiology, speech-language pathology, or both, or a master's or bachelor's degree in one or both areas -Applicant's who have earned a master's or bachelor's degree also must successfully complete a clinical internship in either audiology or speech-language pathology, as applicable, and meet the additional educational requirements described in NJAC 13:44C-3.3. Applicant must have a current business permit in good standing issued by the Board of Pharmacy to operate a pharmacy, -must have a registered pharmacist-in-charge to manage pharmacy, -must be able to meet the requirements of NJAC 8:65-5.4 regarding maintenance of records and drug inventories, and NJAC 8:65-2.1 regarding physical security controls for C.D.S. Applicant must have a active/current professional New Jersey license in good standing. A New Jersey address is required to prescribe, dispense or administer CDS in New Jersey. A cemetery company shall submit annually a notarized report to the Board explaining the extent of , and sources of augmentation to, the maintenance & preservation fund, the designation of the income of the fund as operational expenses, retained income or both, and a list of the securities, bonds, certificates of deposit or other instruments inwhich the corpus of the fund is invested. A chiropractic physician who wish to perform electro-diagnostic tests and special exams must show proof of successful completion of 120 hrs of class & practical hands-on instruction -exam demonstrating competency to perform the pertinent test -substance of the 120hrs of train may be found in the regs cited below -training must be received at a Board-Accredited College, or as part of a postgraduate prog. recognized by counsel of chiropractic education or provided at an accredited medical school or licensed healthcare facility -an utilization of experimental equip. or procedures must be approved by Board. Applicant must be at least 18 yrs. old-good moral character -H.S. graduate -complete 2 yrs./study @ accredited college/univ. -6mths-1st semester any course of study then, next 3 semesters must be in specified area. Graduate from accredited chiropractic college/univ.-achieve passing grade on 2 exams Submit to a criminal history background check. Applicant who failed to renew his/her license within 30 days of expiration date, must pay all past delinquent renewal fees and a reinstatement fee, -must submit affidavit of employment listing every job held during period of license suspension, -if reinstatement sought 5yrs or more beyond expiration, applicant must retake initial licensing exam. Applicant must be at least 18yrs old -Good moral character -H.S. diploma or equivalent -Completion of reporting school program (proof of 1 of the following) -practiced as a court reporter for @ least 3 yrs. of active certificate issued by another jurisdiction or that Natl. Court Reporter Assn. has issued to applicant a certificate a registered merit reporter, certified realtime reporter, certified broadcast captioned or certified communication access realtime translation provider. Applicant must certify that he/she is familiar with & in full compliance with PART 68 of the FCC regulations to be found at 47 C.F.R. Section 68.1 et seq., -must certify that he/she is familiar with NATL. Electrical Code requirements (Article 800-communication circuits) -must certify that he/she will not perform work before obtaining business permit from board, -must certify that the business shall not subcontract telecommunication wiring work to business or person who does not hold a telecommunications wiring exemption issued by the board. Applicant must submit application for a business license, 2 photos, a list of criminal offenses of which he/she has been convicted, -submit to a criminal hist. background record check via fingerprint check, -submit copy of H.S. diploma or equivalent cert. -submit affidavit from prev. employer verifying that requirements found in NJAC 13:31A-2.1, 3.1 or 3.6 as applicable have been met. -for alarm business license, completion of 2hrs train in Barrier Free Subcode, -2hrs train in NJ Uniform Construction Code, -2hrs train in Americans w/Disabilities Act Code, -2hrs train in Industrial Safety and 32hrs relevant trade-related subjects. Same sponsors who have received certification from Intl. Assn. for continuing education & training will be pre approved as sponsors by the committee for trade-related subjects -those without the cert. must submit detailed description of course content & hours of instruction specifying training in area of expertise and pay a fee for each submission of course offerings -sponsors are required to monitor attendance at each approved course & furnish attendees w/verification of attendance. Applicant must be over 18yrs old -Good moral character. -Proof of completion of min. 6mths continuous training within 24mths period entry w/deadline for submitting app. to take next exam. -Proof of successful completion of approved college curriculum in hearing aid selection and fitting -or-having earned a master's degree in audiology from AMER Speech Language Hearing Assn. -accredited college after Jan. 1 1993 -Passing grade on committee -administered practical exam. When committee finds applicant who has license in good standing issued by a state/jurisdiction with requirements for licensure equivalent to or stricter than the program in NJ for determining if an applicant is qualified to dispense and fit hearing aids, the committee may issue a certificate of endorsement. Applicant must be at least 18yrs old -Good moral character -received training and has hadexperience dispensing and fitting hearing aids -temp. licenses are issued for a period that ends 30 days after the conclusion of next exam given after date temp license is issued a temp. license. -may be renewed only for good cause, or if applicant fails next exam given -only one renewal permitted. Applicant must be of good moral character -hold a H.S. diploma or equivalent -1st path to licensure is to complete 140hrs of education at a committee-approved Home Inspectors School and 40hrs of being mentored by a NJ licensed home inspector -must provide proof of having obtained insurance -must pass Natl. Home Inspector Exam (EBPHI) -2nd path to licensure for those who have not passed approved course of study but who completed at least 250 fee-paid home inspection under direct supervision of a NJ licensed home inspector -applicant also must obtain insurance & pass EBPHI EXAM. Applicant must be at least 18.-hold a H.S. diploma or it's equivalent -must have passed either the ASHI exam or the Natl. Home Inspection Exam (EBPHI) -must submit proof that prior to 12/30/2005, he/she performed 400 home inspectors or 300 home inspections in the 3yrs period immediately preceding 12/30/05-must submit documentation that applicant was performing fee-paid home inspections for at least 3yrs prior to 12/30/05 -also must obtain required insurance. Applicant must be a Civil Engineer currently licensed by the NJ State Board of Professional Engineers and Land Surveyors -applicant also must obtain an errors & omissions insurance policy in min. amount of one-half million dollars. Applicant must be an Architect who holds a license issued by the NJ State Board of Architects -applicant also must submit proof, along with the application to become a licensed home inspector, the he/she has the requisite training, education and experience to be a home inspector (see NJAC 13:27-4.12(b) for a complete list of subject areas applicant must have knowledge of). Applicant must pay all delinquent renewal fees and a reinstatement fee -must submit 1 of following: (a) proof of certification/licensure in any other state or jurisdiction where applicant worked as home inspector during period of NJ license suspension (b) affidavit that applicant worked as a Home Inspector in state/jurisdiction that does not require licensure during period of NJ license suspension, or (c) affidavit applicant has not worked as a Home Inspector in any jurisdiction during period of NJ licensure suspension. (a) For purposes of this section, "practical hands-on experience" means experience in the installation, maintenance, extension, alteration, repair and removal of piping, plumbing fixtures, plumbing appliances and plumbing apparatus. Practical hands-on experience does not include time spent in performing managerial tasks, such as supervising, engineering, estimating, Or time spent performing tasks, which do not constitute the practice of plumbing as defined in N.J.A.C. 13;32-1.3.
(b) Applicants for examination for a license as a master plumber shall present proof to the Board that he or she: 1. Has completed a four-year apprenticeship program approved by the United States Department of Labor and has completed one year of practical hands-on experience as a journeyman plumber; Or

A trainee permit may be renewed a maximum total of 3 times upon submission to the Board of a renewal app., the permit renewal fee, and a log in the form set forth in NJAC 13:40A-4.7. Filing fee $75.00
NJ State Board of Social Work Examiners now must issue to you an exam authorization in order for you to schedule a date with the ASWB (Association of Social Work Boards) to take the “Clinical” level exam. You are required to complete and return the enclosed “Pre-authorization” form to the Board office. Please refer to specific instructions on the authorization form regarding New Jersey LSWs and out-of-state applicants.

Applicant must complete application form,-pay app. fee, -must have earned a baccalaureate degree in social work from an educational program accredited by, or on candidacy for accreditation, the C.S.W.E., -must submit to criminal hist. background record check. For additional requirements and qualification see: NJSA 45:15BB-6c and NJAC 13:44G-4.3.

Applicant must complete application form,-pay app. fee, -must have earned a master's degree in social work from an educational program accredited by, or on candidacy for accreditation, the C.S.W.E. or doctorate in social work from an accredited college/univ. -proof of successful completion of master's level exam administered by the A.S.W.B., -must submit to criminal hist. background record check. Applicant must complete application form,-pay app. fee, -must have earned a master's degree in social work from an educational program accredited by, or in candidacy for accreditation, the C.S.W.E. or doctorate in social work from an accredited college/univ. -must have completed 12 sem. hrs of graduate level course work in methods of social work practice, exclusive of field work (see NJAC 13:44G-4.1(b)ii(1)thru(4) for details re: required area of study) -must provide supervisor's certification that applicant has acquired 2yrs of satisfactory social work experience under supervision in not less than 2 or not more than 3yrs while licensed as an L.S.W. -must pass exam admin. by A.S.W.B. -must submit to criminal hist. background record check.
