New Jersey Prescription Blanks (NJPBs) Information:

On February 18, 2014, the Division of Consumer Affairs adopted new regulations which will incorporate print-based security features into all New Jersey Prescription Blanks (NJPBs).

August 18, 2014

The "old" NJPBs may be used by practitioners for validly written prescriptions dated on or before August 18, 2014.

After this date, practitioners must use the new NJPBs to issue prescriptions written for their patients.

Note: Patient prescriptions originally written on or before August 18, 2014 will be filled at a New Jersey pharmacy pursuant to the guidelines set forth in N.J.A.C. 13:39-7.3 for non-Controlled Dangerous Substances (non-CDS) and N.J.A.C. 13:45H-7.5 for Controlled Dangerous Substances (CDS).
