LSRP Complaint Form with Instructions

The Professional Conduct Committee of the Site Remediation Professional Licensing Board held an Open Forum on Tuesday, February 14, 2012 at 3:00 p.m. to present the latest disciplinary flowchart and to obtain feedback from interested individuals. Thereafter, the Professional Conduct Committee revised the flowchart, which was approved by the Board at the March 5, 2012 meeting. Revised Disciplinary Flowchart
