RESPONSE CARD TO APPLICATION FOR ADMISSION

 

POLICY STATEMENT.

The Division of Veterans Healthcare Services (DVHS) and the New Jersey Veterans Memorial Homes (VMH) acknowledge that applicants to a VMH are comprised of New Jersey veterans, the spouses of New Jersey veterans, the surviving spouses of New Jersey veterans, Gold Star parents, Gold Star widows/widowers, and allied veterans (as defined in N.J.A.C. 5A:5). These applicants are one of our primary customer groups, and are therefore entitled to outstanding customer service. An important part of outstanding customer service is good communication. Consequently, a process will be established to communicate with the applicants and/or their family members/ power of attorney to notify them that the VMH has received their Application for Admission. A standard “Response Card” process will be implemented for all three VMHs to use when communicating with applicants.

PURPOSE.

To ensure that applicants to a New Jersey Veterans Memorial Home (VMH) and/or their family members/power of attorney are notified when their Application for Admission has been received by the VMH.

PROCEDURE.

  1. When an Application for Admission to a New Jersey Veterans Memorial Home (VMH) is received by the VMH, a designated staff member at the VMH will obtain a Response Card from the Admissions Officer.

 

The staff member will write the date that the Application was received by the VMH on the date “line” located above the text portion of the response card.

    1. The response card will then be folded in half, and sealed with either tape or a staple.

 

    1. A check mark will be placed in the box next to the appropriate VMH name and address.
    1. The applicant’s/family member’s/power of attorney’s name and address will be written on the address lines.

 

    1. The response card will have postage affixed according to VMH protocol, and will then be mailed to the applicant/family member/power of attorney.
