ADMISSIONS COMMITTEE

 

POLICY STATEMENT:

The Division of Veterans Healthcare Services requires each of the New Jersey Veterans Memorial Homes to establish manuals of policies and procedures for providing services. Procedures shall permit the organization of committees, as needed, to ensure compliance with the New Jersey Standards for Licensure of Long Term Care Facilities, and with applicable Federal and New Jersey State Administrative Codes, related laws, regulatory and licensing standards.

PROCEDURE:

A. New Jersey Veterans Memorial Homes

  1. The New Jersey Veterans Memorial Homes shall establish an Admissions Committee charged with the responsibility for reviewing applications made for admission to the homes, and for rendering determinations for placement in compliance with all applicable Federal and State laws, rules, and regulations. N.J.A.C. Chapter 5A: 5-1.1 through 5A: 5-6.1 inclusive shall serve to guide the committee in handling applications for admission.
 

a. Admissions Committee membership shall be representative of the following departments/designees:

Administration
Director of Social Services (Chairperson)
Medical Director
Director of Nurses
Quality Improvement Coordinator
(Others as deemed necessary)

b. The Admissions Committee will meet weekly, or more frequently if necessary, to address the following:

 

1. Establish a single waiting list, updated on a regular basis, of all individuals who have applied for admission to the facility.

2. The waiting list shall reflect, in chronological order, the full name and address of the individual applying by the date the written application for admission is made (waiting list in process).

3. The waiting list shall reflect that admissions are offered in the order in which the application was considered “complete” in the chronological order (date) in which all criteria is met.

4. The waiting list shall support any valid reason why the individual whose name appears first on the list is not admitted to the facility.

5. The facility shall not deny admission to any applicant for admission based upon diagnosis or health care needs, if the applicant’s health care needs can be reasonably accommodated without reducing the quality of care provided to other residents, and are commensurate with the services provided by the facility.

6. Whenever the facility denies admission to an applicant for admission, the facility, within 14 days of the denial, shall provide written notice of the denial and the reasons therefore to the applicant or person applying on the applicant’s behalf.

 

i. A record of each completed application, including the disposition and stated reason if admission is denied, shall be kept for one year.
