MEDICAL SERVICES INITIAL ASSESSMENTS

POLICY STATEMENT:

The Division of Veterans Healthcare Services requires each of the New Jersey Veterans Memorial Homes to establish a Medical Services Program that assures veteran home residents receive medical care beginning on the day of admission.

PURPOSE:

This policy and procedure shall serve to ensure a new resident admitted to one of the New Jersey Veterans Memorial Homes receives medical services beginning on the day of admission, conducted by a physician who shall provide orders for care.

PROCEDURE:

A. New Jersey Veterans Memorial Homes

1. The New Jersey Veterans Memorial Homes shall assure each veteran home resident receives care and treatment interventions, to include nursing care considered routine by standards of medical practice, beginning on the day of admission, which includes:

a. The Medical Director shall designate a primary care physician/advance practice nurse and an alternate physician who can be contacted when necessary.

b. A physician/advance practice nurse shall provide orders for each resident's care beginning on the day of admission.

c. A physician shall examine each resident within five days before, or 48 hours after, admission.

d. A primary care physician shall see the resident at least once every 30 days for the first 90 days after admission, and at least once every 60 days thereafter. After the first 90-day period, alternate 30-day visits may be delegated by a physician to a New Jersey licensed physician assistant or advance practice nurse, in accordance with facility policies.
