NOTIFICATION OF RESIDENT'S CONDITION
POLICY STATEMENT:
The Division of Veterans Healthcare Services requires each of the New Jersey
Veterans Memorial Homes to establish a Medical Services Program that provides
the veteran home resident/healthcare representative a full description of
the medical condition/plan of treatment.
PURPOSE:
This policy and procedure shall serve to ensure veteran home residents,
if competent, or the selected healthcare representative, if the resident
is incompetent or it has been determined inadvisable to inform the resident
of their medical condition, a full description of the medical condition/plan
of treatment. The physician/ advance practice nurse may delegate the notification
authority to a licensed professional, depending on the needs of the resident
and the situation.
PROCEDURE:
A. New Jersey Veterans Memorial Homes
 |
1. The New Jersey Veterans Memorial Homes shall assure each veteran
home resident/healthcare representative receives a complete, full description
of the medical condition/plan of treatment interventions. |
B. Competent Residents

1. The physician/ advance practice nurse must
inform the resident of changes in their medical status / condition
and treatment plan.
2. Documentation of changes must be recorded
in the medical record.
3. The physician/ advance practice nurse
must record the significant change(s) in status in the
medical record
containing the following information: |
a. Date and time of communication;
b. To whom the information was
provided; and
c. What information was shared/conveyed.
C. Incompetent Residents

1. The physician/ advance practice
nurse must inform the health care representative of any significant
change in status/condition and treatment plan.
2. Documentation of changes must be recorded
in the medical record.
3. The physician/ advance practice nurse must
record the significant change in status in the medical record
containing the following information: |
a. Date and time of communication;
b.
To whom the information was provided; and
c. What information was
shared/conveyed.

4. The health care representative will be notified
of the resident's change in condition within 24 hours, when there
is: |
a. An accident or
injury to the resident requiring hospitalization.
b. A significant change
in medical status or the resident becomes critically ill.
c. A transfer
to, or return from, the hospital.
d. A permanent transfer or discharge
out of the facility.
