POLICY STATEMENT.

The Division of Veterans Healthcare Services (DVHS) requires each of the New Jersey Veterans Memorial Homes (VMH) to review applications for admission and related pertinent information, via their respective Admissions Committee (ref: Divisional Policy and Procedure, Communications - 13-02-001), for the purpose of rendering determinations for appropriate nursing unit and room placement.

PURPOSE.

This policy and procedure serves to ensure the New Jersey Veterans Memorial Homes, via the Admissions Committee review process, assess all data on applicants for admission to ensure each receives unit and room placement consideration appropriate to attain and maintain the highest practicable level of optimal independence and care services.

PROCEDURE.

  1. The New Jersey Veterans Memorial Homes, via Admissions Committee practices, will assess all data on applicants for admission and decide upon the initial unit and room assignment, assuring that placement for each resident promotes the attainment and retention of the highest practical level of independence and care services.
  1. Admissions Committee

 

  1. Private room assignments will be based on the individual resident's medical needs. The private room assignment is to be made in conjunction with the Interdisciplinary Team.
  1. Unit and room assignments, requests for unit and/or room transfers, etc. following initial placement, will be addressed using the Interdisciplinary Team approach.

 

  1. If a resident is transferred to an outside facility for medical attention, a bed will be held available for the resident’s return. The bed may not be the originally assigned bed, but will be a bed somewhere within the VMH.
