CONFIDENTIAL OPINION SURVEY
PURPOSE:
The Confidential Opinion Survey affords the residents, family members
and resident representatives the opportunity to express their
satisfaction, suggestions regarding knowledge of residents' health
status, treatments, knowledge of facility policy and staff members'
roles.
PROCEDURES:
A. The Confidential Opinion Questionnaire will be made available to
residents, family members and resident representatives annually.
B. The Questionnaire will be reviewed upon return, and all areas of
concerns will be explored with the appropriate provided.
C. Responses received will be tabulated and forwarded to the various
facilities for follow up corrective action.
D. At the request of any customer, a written response will be completed
and forwarded within fourteen (14) days.
