FILE IDENTIFICATION

 

PURPOSE:

The identification number maintains confidentiality of medical information and also assists in the retrieval of medical records when needed for reference.

DEFINITION:

The "Numbering System" is a five digit identification number first assigned at the time of admission of the veteran home resident to the facility. If the veteran home resident is transferred to another facility beyond thirty days, a code letter is added to the original identification number to indicate the number of hospital admissions that involved an extended stay.

PROCEDURE:

Medical record numbers are assigned as follows:

A. New Admissions

All medical records have a five digit identification number. The first two digits of the number represent the year, and the last three digits are chronologically assigned as individuals are admitted to the facility throughout the year.

Example:

88-032

 

88 = Year admitted
032 = 32nd individual admitted during 1988.

B. Temporary Transfers

Individuals transferred to another facility beyond thirty days will have a new chart initiated. The chart number is to remain the same except for a code letter to be added indicating the number of hospital admissions.

Example:

88-012c

 

86 = Year admitted
012 = 12th individual admitted during 1986.
c = The individual had three hospital admissions beyond thirty days.
