FILING MEDICAL RECORDS
PURPOSE:
To ensure that all medical data on a veteran home resident is located in
one area so it is readily accessible to those who need it as reference
in providing continuity of care, to prevent the loss or destruction of
valuable information, and to promote uniform organization amongst all
medical record filing equipment.
DEFINITION:
"Filing" is the arrangement of records one behind the other for
preservation and reference.
PROCEDURE:
A. All medical records are located in the Medical Records Office, and
are filed in numerical order according to the admission number of the
individual assigned to them at the time of admission to the facility.
B. The medical record of individuals transferred to another facility
beyond thirty days are to be filed first, in numeric order then, in
alphabetic order according to the code letter indicating the number of
hospital admissions.
