INDEXING
PURPOSE:
To record all diseases encountered by a veteran home resident during
their stay within a NJ Veterans Memorial Home Facility.
DEFINITION:
A Disease Index lists diseases and conditions according to the
International Classification of Diseases - 9th edition - Clinical
Modification and is assigned by Medical Record Department personnel.
PROCEDURE:
Medical records shall be indexed by disease with the disease card filed
in numerical order. Cross indexing is utilized with reference to the
code numbers for all diseases a veteran home resident may have had
during their stay within a facility for each separate entry posted in
the index.
Principal diagnosis is designated by an asterisk.
