TICKLER FILE
PURPOSE:
A "Tickler File" shall be maintained to ensure that deficiencies are
corrected and charts are completed within ten (10) days of notice of
discharge/death.
DEFINITION:
A "Tickler File" is a card file used to serve as a reminder of important
dates.
PROCEDURE:
The following items are a detailed list of the content, organization and
operation of a "Tickler File."
A. Content of "Tickler File"
1. The "Tickler
File" is composed of index cards containing the following items.
 Resident
Name
 Resident
Number
 Admission
Date
 Discharge/Deceased
Date
 Month
of Audit
 Date
Deficiency is Noted
 Date
Memo is Sent
 Date
Correction is noted
 Departments
with Deficiencies
B. Organization of a "Tickler File"
The "Tickler File" is arranged by:
1. Nursing
Stations/Wings/Floors
2. Deficiencies
 a.
To be completed within ten days from the date on the memo sent to
departments with deficiencies.
3.
Discharged/Deceased
 a.
To be completed within ten days of discharge or death.
C. Operation of a "Tickler File"
Each step is to be followed when a medical record is deficient or when a
resident has been discharged/deceased:
1. Deficient
 a.
When a medical record is deficient, the following items are to be
completed:
  (1)
date deficiency is noted
  (2)
date memo is sent
 b.
The index card is filed behind the divider labelled "Deficiencies."
 c.
In ten days, all index cards filed behind "Deficiencies" are to be
reviewed for corrections.
 e.
When all corrections are documented, the auditor is to record the date
the correction is noted on the index card and re-file it under the
correct nursing location.

Discharged/Deceased

a. When a resident is discharged/deceased, the
date is recorded on the index card and it is filed behind the
divider labeled "Discharged/Deceased."
b. In ten days, all index cards filed behind
"Discharge/Deceased" are to be reviewed for correction of
deficiencies and completion of reports.
c. When all corrections are documented and all reports
completed, the auditor can discard the index card. |
