COMMUNITY RESOURCES

POLICY STATEMENT.

The Division of Veterans Healthcare Services requires that each of the New Jersey Veterans Memorial Homes’ Social Services employees be well informed regarding the availability of community resources for the purpose of coordinating the delivery of services, via referrals, for residents and their families.

PURPOSE.

This policy and procedure shall serve to ensure New Jersey Veterans Memorial Home residents and their families receive appropriate referrals.

PROCEDURE.

A. Social Services:

1. All Social Workers will be oriented and maintain familiarity with the manuals, directories, and resource literature. Social Services staff members will maintain and update these resources on a regular basis.

2. Professional staff of the department will orient themselves to resources in the community and state, and acquire their own resource manuals.

3. The Director of Social Services will familiarize department staff with regard to the major organizations, agencies, and medical facilities to which the residents are referred. Departmental staff will utilize and develop the index of resources and contact persons.

4. All professional staff of the department will make themselves available to provide information and referral services to residents and families.

5. Social Workers will contact appropriate community agencies and coordinate the delivery of services.
