VERIFICATION OF CNA CREDENTIALS

I. PURPOSE:

All Nurse Aides employed in Long-Term Care Facilities are required to meet criteria established by the Department of Health, in accordance with NJAC 8:39-43.20. No licensed Long-Term Care Facility shall employ a person as a Nurse Aide without making inquiry to the NJ Nurse Aide Registry and to any other State Nurse Aide Registry in which the facility has a good faith belief the perspective Nurse Aide is registered. The facility shall verify that the individual seeking employment as a Nurse Aide possesses a valid Nurse Aide certification.

II. POLICY:

A. If the Nurse Aide applicant is considered for employment by the facility, the Personnel Officer/designee will contact ASI to verify that said Nurse Aide possesses a current and valid CNA certification and is in good standing on the NJ Nurse Aide Registry.

B. The Personnel Officer/designee shall require the applicant to present the original CNA Certification for verification.

C. The Personnel Officer/designee will utilize the Certified Nurse Aide Information Form (DFF-43-1) to verify information from ASI. The telephone and facsimile verification forms will reflect applicant's name, social security number, certification number and date of certification.

D. ASI will provide the Personnel Officer/designee with immediate information on an applicant's status in telephone verifications and in the case of facsimile verification, they will provide verification within 48 hours.

E. Records of verification of applicant's status will be kept on file in the Personnel Office.
