TRAINING ROSTER FOR IN SERVICE & OUT SERVICE
I. PURPOSE:
To establish accurate documentation of all staff training records within
the facility.
II. PROCEDURE:
A. Whenever any relative (pertinent) training is done by any person(s)
in a facility, a Department Training Roster is to be used. For
Outservice Training, a Roster also has to be completed by the
individual(s) attending the program. The original copies of all the
training rosters are to be returned to the Employee Development Office.
B. The following information is required in order to process the
Training Roster:

1. Course Title
2. Subject
3. Instructor
4. Title of Instructor
5. Date the course is presented.
6. Number of class hours (to the nearest half hour).
7. Total number of people trained.
8. Last name of each attendee is to be printed first and the
first name is to be printed last.
9. Social Security Number of the attendee has to be documented.
10. Civil Service Title of the attendee.
11. Shift hours worked.
12. Sex.
13. Affirmative Action Code (only one number is to be used) |
C. The information on the training
roster is all coded for computer input to keep accurate records of all
individual staff's training records.
D. All the above information is tabulated in the computer for storage
and retrieval purposes, if and when needed for reporting.
E. All the information which is obtained is utilized for
the following:

1. All training information completed by
Department of Military and Veterans Affairs Employee becomes
part of a permanent training record.
2. Training documentation may be audited by the Veterans
Healthcare Service, Department of Health and Senior Services
surveyors, facility Quality Improvement Coordinator,
supervisors, and the employee himself/herself. |
F. It is essential for each staff person at the facilities to be aware
of all training that he/she has received. Staff is asked to submit
training records from other facilities that staff have worked at in
order to update all training records.

1. The Employee Development Department keeps a
separate training file on each staff member. |
G. Annually, the Employee Development Department prepares a computerized
summary of all the training the individual staff has to date. If there
are any errors, time is afforded staff to alert the Employee Development
Department and any necessary corrections are made.
H. If a staff member leaves the
facility, the training file on the individual is maintained by the
Employee Development Department. If the facility where the person
transfers requests a summary training, the information is sent to the
Director of Personnel and/or Director of Employee Development at the new
facility.
