Program Description
Funds are available to help persons with physical disabilities purchase items for basic care or mobility, that are not covered by insurance, Medicaid or other sources.
 
Fees
No fee for qualified individuals
 

Eligibility

General Requirements:
A NJ resident
Has a severe, chronic disability that meets all of the following:
Is attributable to a mental or physical impairment or combination of mental or physical impairment, other than mental illness;
and Is manifested before the age of 22; and Is likely to continue indefinitely;
and results in substantial limitations in at least 3 or more of the following 6 major life activities (For an individual under age 18, the following factors shall be evaluated according to expectations based upon the individual's age.) Self-care, Making decisions, Living independently, Learning, Earning a living, Mobility, Communicating
