| Assisted Living (AL) offers a range of personal and health care services, 24 hours a day, in a more home-like setting. AL offers residents a way to "age in place" as their care needs increase and is a possible alternative to nursing home placement. New Jersey subsidizes assisted living placements of eligible low-income older adults and adults with disabilities through a Medicaid Home and Community Based waiver program.
In AL, residents can receive: care management, help with personal care, nursing services including some skilled nursing care, ongoing health monitoring, pharmaceutical services and help taking medications, social work services, dietary services, recreational activities, homemaker and chore services, and transportation. In addition, participants are eligible to receive certain services covered under the New Jersey Medicaid State Plan, such as physician visits and hospital coverage.
To be eligible for the AL waiver, participants must meet both financial and clinical eligibility criteria.
