Program Description
CAP is a Medicaid Waiver that offers several care alternatives to individuals who would otherwise be at risk of placement in a nursing facility. Services available are:
  • Care Management
  • Respite Care
  • Homemaker 
  • Environmental Accessibility Adaptations
  • Personal Emergency Response System
  • Home-Delivered Meals
  • Caregiver/Recipient Training
  • Social Adult Day Care
  • Special Medical Equipment and Supplies
  • Chore Services
  • Transportation
  • Attendant Care
  • Home-Based Supportive Care

Additionally, individuals in CAP are eligible to receive services covered under the New Jersey Medicaid State Plan, such as Adult Day Health, pharmaceutical, physician visits and hospital coverage.

 
Fees
No fees for eligible individuals.
 

Eligibility

General Requirements:
Individuals must be determined clinically eligible for nursing home level of care.
Individuals must be 65 or over, or between 21 and 64 and determined disabled by SSA or the Division of Medical Assistance and Health Services, Disability Review Section.
Individuals cannot be enrolled in other Medicaid waivers.
 
Income Requirements:
Qualifies for SSI in the community; or
Qualifies for Medicaid Only (gross monthly income at or below $1,869, resources at or below $2,000); or
Qualifies for New Jersey Care (gross monthly income at or below $817, and resources at or below $4,000)
