Program Description
Integrated Case Management Services (ICMS) are consumer-centered services provided predominantly off-site in the consumer's natural environment. These personalized, collaborative, and flexible outreach services are designed to engage, support, and integrate consumers.

Services available are:
  • assessment
  • individual counseling
  • group counseling
  • medication education/monitoring
  • referral to medical service
  • referral to social service
  • referral to educational service
  • referral to vocational service
  • referral to housing service
  • case management services
  • links to entitlement services
 
 
Fees
No fee for qualified individuals
 

Eligibility
General Requirements:
Individuals 18 years of age or older who are severely and persistently mentally ill
 
Income Requirements:
No individual otherwise eligible shall be denied admission due to inability to pay or the type of insurance held.
