Program Description
The CCPED Medicaid waiver program provides services in an individual's home rather than in a nursing facility or institution.  Eight services are available under CCPED: care management, home health services, homemaker, adult day health, non-emergency medical transportation, respite care, social day care and prescription drugs.  All CCPED participants must enroll in Medicare Part D or have other prescription coverage.

Eligibility

General Requirements:
Individuals must be determined clinically eligible for nursing home level of care.
Individuals must be 65 or over; or between 21 and 64 and determined disabled by SSA or the Division of Medical Assistance and Health Services, Disability Review Section and be eligible for Medicare or have other health insurance.
They cannot be eligible for Medicaid or enrolled in other Medicaid waivers.
 
Income Requirements:
Medicaid Only - Institutional Level - Gross monthly income above SSI, and up to $1,869, but with resources less than $2,000 (individual) or $3,000 (couple).
