Program Description
Provides diabetic eye disease screening and medical referral for corrective or preventive eye services for individuals* with diabetes.
(*A child must have been diagnosed with diabetes for five years or more before becoming eligible for participation in this program.)

Eligibility

General Requirements:
Uninsured or underinsured persons* diagnosed with diabetes
Do not have a regular source of eye care
Have not had a dilated eye exam within one year

Income Requirements:
Income limit is dependent on the number of individuals in the family unit. The Commission for the Blind and Visually Impaired uses 300% of the federal poverty guidelines for the income criteria. Income limits for a family size of one would be $28,710; for a family size of two, it would be $36,490; for a family size of three, it would be $48,290; and for a family of four, it would be $58,050. The Department of Health and Senior Services awards grants to the Commission for the Blind and Visually Impaired to provide, through mobile services, diabetic eye disease screening and medical referral for corrective or preventive eye services for individuals with diabetes.
