Family Centered Care Services programs focus on providing family-centered, community-based, individualized and accessible services to children up to 21 years of age with a broad range of disabilities or chronic illnesses, and to women, infants, children, youth and their families living with HIV and AIDS.
The SCHS Case Management Program in conjunction with the County Boards of Chosen Freeholders administers a system of 21 case management units. Nurses and social workers at each county unit help families with children with special needs to get connected with local, State and/or federal programs and services. The SCHS Case Management Program also helps eligible families to purchase braces, prosthesis, hearing aids, and pharmaceutical needs for children with asthma and cystic fibrosis.
The Specialized Pediatric Services Program administers grants to 11 Child Evaluation Centers including 6 Fetal Alcohol Syndrome (FAS) Diagnostic Centers, 5 Cleft Lip/Palate Craniofacial Centers and 3 Tertiary Care Centers to help New Jersey families get access to in-State pediatric specialty care.
The Family Centered HIV Care Network of seven regional centers (partially funded through the Ryan White Care Act) helps to assure that New Jersey families have access to comprehensive specialty care to treat HIV and AIDS. |
