RN/LPN Endorsement Application Process

If you live, work or attend school within 10 miles of the State's borders you will be sent information regarding the criminal history background check after you submit a completed application. Please click on the "RN/LPN Endorsement Applicaton Packet" link below to proceed to the application. (For a complete listing of ZIP codes that are within the 10-mile limit, please visit:

If you live, work or attend school 10 or more miles beyond New Jersey's borders, you have the option of using manual fingerprint cards in order to satisfy the fingerprinting requirement. Please enter your personal information in the fields below so that we may expedite the RN/LPN Endorsement process for you. While you take the time to fill out your application in its entirety, we will be sending you the information necessary for the fingerprinting portion of the criminal history background check process.

Please send all pertinent materials to the New Jersey Board of Nursing at the same time to prevent any delays. Once you have filled in all the necessary fields below, click "Submit" so that your information can be sent directly to the New Jersey Board of Nursing. You may then click on the link for the "RN/LPN Endorsement Application Packet" on the confirmation page and you will be sent directly to the application.
