The Unified Provider Directory is for use by members of the New Jersey State Health Benefits Program (SHBP), and their covered dependents in identifying physicians and other health care professionals, suppliers, and facilities who are affiliated with one or more of the medical benefits plans available under the SHBP.

The Unified Provider Directory lists providers in New Jersey, Southern New York, and Eastern Pensylvania.

The information in thw Unified Provider Directory is subject to change. Contact the health professional or supplier, or your health plan, to verify the continuing participation of any provider in the health plan before seeking their services. If enrolled in a HMO plan, you must follow the HMO's rules regarding a change in primary care physician, seeing specialists, etc. Failure to follow your health plan's rules will result in the denial of payment as in-network care for the services received. In order to avoid such events, you should call your plan's member services office to: change your primary care physician, confirm the authorization of a referral to a specialist, and verify that any hospital admission has been approved.
