Customers may order forms over
the telephone by calling (609) 292-9292 and following the prompts to reach
the forms line. The forms line enables customers to order most corporate
filing forms and forms for Notary Public, Collection Agency Bond , and
Trade Mark applications.
Annual Report Forms may only
be obtained by writing to PO Box 302, Trenton, NJ 08625 or by faxing your
request to (609) 984-6849. Remember to include the following information
in your request: Return Address; Name of the Business; and the 10 digit
ID # (if Known).
UCC forms are not provided
by the State. These forms are widely available from Stationers' stores.
