CP&P Form 11-18, Parental Consent for Operation, Treatment or Diagnostic Work-Up

PURPOSE AND USE

 

This form is used to transfer from the parent or legal guardian to the Division of Child Protection and Permanency, the authority to consent to any operation, treatment, or diagnostic work-up required by the child while placed in resource care or residential care.

 

If the Consent to Independent Living By Parent(s), Legal Guardian, or Legal Custodian, CP&P Form 10-8, or the Residential Placement Agreement, CP&P Form 25-59, has been signed by the parent or legal guardian as part of the placement plan, the CP&P Form 11-18 or 11-18(S) is not necessary in an emergency or for routine care since the authorization is already contained in those agreements.  However, in those situations where a parent can be contacted in person prior to operation, treatment, diagnostic work-up, the parent or guardian should be asked to sign.

 

INSTRUCTIONS FOR COMPLETING THE FORM

 

The Worker completes the form by hand and in duplicate and the parent or legal guardian signs it.
