WHEN TO USE IT
Use the CP&P Form 14-149, Consent of Custodian to Adoption, to give consent to the adoption of a child who has been placed in an adoption home on the basis of CP&P Forms 14-86, Surrender of Custody and Consent for Adoption, 14-86a, Affidavit of Surrendering Parent, 14-86b, Affidavit of Local Office Manager, and 14-86d, Affidavit of Local Office Supervisor. See CP&P-IV-C-1-600.
HOW TO USE IT
• The assigned Worker prepares two copies of the form using the template in the computerized Forms Manual and includes the forms in the Consent package for the Local Office Manager.
• The Local Office Manager approves the adoption of a child by signing both copies of the form, after he or she reviews the Consent package in preparation for finalization of the child's adoption.
• Both copies of the form must be notarized.
TIPS FOR COMPLETING THE FORM
• If a single parent adoption, enter "Not Applicable" in the text field for the name of Adoptive Parent 2.
• If a designee has been appointed in the Local Office Manager's absence, the designee signs his or her own name on the signature line on both copies of the form for the Local Office Manager followed by "for" and the name of the Local Office Manager. On the next line, enter the name of the Local Office Manager. Note: This authority should not be delegated below the Local Office Manager level on a regular basis.
