WHEN TO USE IT

 

Use the CP&P Form 14-179, Pre-adoptive Family Care Agreement between the New Jersey Division of Child Protection and Permanency and Preadoptive Parent(s), to:

 

·         Clarify the child's pre-adoptive status;

·         Identify the risks involved in a pre-adoptive placement; and

·         Facilitate the process of advising the preadoptive parent(s) of the conditions of the pre-adoptive placement.

HOW TO USE IT

 

·         The assigned Worker prepares three copies of the form using the template in the computerized Forms Manual.

·         During the pre-placement interview with the preadoptive parent(s), the assigned Worker and Resource Family Worker explain:

-       The child's unique legal status;

-       "Legal risks"; and

-       What needs to be done to obtain legal clearance for adoption.

·         After the interview, when all parties agree to the plan, both Workers and the preadoptive parent(s) sign the three copies of the Agreement.

TIPS FOR COMPLETING THE FORM

 

If a single parent adoption, enter "Not Applicable" in the text field for the name of Preadoptive Parent 2.
