WHEN TO USE IT

 

The CP&P Form 14-211 is signed by the non-adopting resource parent(s) who cared for a child for six months or one half of the life of the child, whichever is less, in the two years prior to an adoption complaint being filed for the child.

 

The letter is given or sent to the former resource parent when the child has been removed from the resource home and is being placed in a selected adoption home or at the time of litigation.

HOW TO USE IT

 

Prepare the form using the template found in the on-line Forms Manual (11-20).

 

Print the form on Local Office letterhead.

 

Send or give the letter to the resource parent when the child has been removed from the home or at the time of litigation.

 

The former resource parent signs and dates the waiver and returns it to the child's Worker.
